ABSTRACT

The goal of this study was to evaluate by endoscopy, possible intercurrences during the sinus floor lifting employing the Summers technique, besides verify the implants' survival rate after 10 years. Six patients (12 sinus) were included in this study. The same surgeon performed the procedures, under local anesthesia and venous sedation in the hospital. All participants underwent sinus lift and implant placement in only one procedure, as previously planned, using endoscopic analysis (Stortz®). All cases received bovine bone graft (Bio-Oss®) before the implant placement. After 10 years, the patients were recalled for follow-up. Two intercurrences (16.66%) were detected using the endoscope, one simple rupture, and another perforation with the leaking of the graft within the sinus. Both were reverted and corrected immediately. There was one implant loss (8.33%), therefore this patient did not undergo any intercurrence in transoperative, and the membrane was elevated lesser than 5 mm. The survival rate reached was 91.66%. The osteotome technique constitutes a reliable method with a long-term of 10 years presenting a high implant survival rate, suggesting an elevation up to 5.5 mm in healthy patients. The occurrences in transoperative were only detected by the endoscopic analysis which must be stimulated to guarantee more secure visibility. Otherwise, the association the atraumatic technique and endoscope was tough, increased the costs, limiting the use routinely.


O objetivo deste estudo foi avaliar através de endoscopia as possíveis intercorrências durante levantamento de seio maxilar fechado, além de verificar a taxa de sobrevivência dos implantes após 10 anos. Seis pacientes (12 seios maxilares) foram incluídos neste estudo com idades entre 26 e 74 anos. O mesmo cirurgião realizou os procedimentos, sob anestesia local e sedação venosa em ambiente hospitalar. Todos os participantes foram submetidos à elevação do seio nasal e colocação do implante em apenas um procedimento, conforme planejado anteriormente, por acompanhamento transcirúrgico da endoscopia (Stortz®). Todos os casos receberam enxerto ósseo bovino (Bio-Oss®) antes da colocação do implante. Após 10 anos, os pacientes foram chamados para acompanhamento. Foram detectadas duas intercorrências (16,66%) com o endoscópio, uma ruptura simples e outra perfuração com extravasamento do enxerto para dentro do seio. Ambos foram revertidos e corrigidos imediatamente. Houve perda de um implante (8,33%), portanto esse paciente não apresentou intercorrência no transoperatório e a membrana estava elevada menos de 5 mm. A taxa de sobrevivência alcançada foi de 91,66%. A técnica do osteótomo constitui um método confiável em longo prazo (10 anos) apresentando uma alta taxa de sobrevivência do implante, sugerindo que uma elevação de até 5,5 mm em pacientes saudáveis é possível. Assim, verificou-se que as ocorrências no transoperatório foram detectadas apenas pela análise endoscópica que deve ser estimulada para garantir uma visibilidade mais segura. Por outro lado, a associação da técnica fechada com o endoscópio foi difícil, aumentou os custos, limitando seu uso como rotina

OBJECTIVE@#To explore the feasibility of immediate implantation after tooth extraction in the maxillary molar socket with poor bone quality beneath the sinus.@*METHODS@#We collected the data from the patients undergoing extraction of maxillary molars with poor bone quality between the sockets and sinuses. Sinus lifting and immediate implant following the extraction were performed simultaneously in these cases, and the primary stability of the implants, wound healing, and changes of the sinus were observed. At 6 months after the operations, the crowns were installed on the implants. The masticatory function was observed, and the growth of the alveolar bones and their changes after the operations were examined using microcomputed tomography (MCT).@*RESULTS@#We analyzed 32 extraction cases with immediate implantation in the maxillary molar sockets with poor bone quality beneath the sinus. The average age of the patients was 59.8 years, and the length and diameter of the implant ranged from 8.5 to 10 mm and from 4.5 to 5.5 mm, respectively. The torque force of the implants varied from the minimum (in which cases the implants remained fixed after insertion with fingers) to the maximum of 30 N·cm. The postoperative recovery was uneventful in all the cases and no failed or movable implants were found. At 6 months after the operation, none of the patients showed abnormalities in the sinus, and in all the cases the crowns were successfully installed on the implants with good recovery of the masticatory functions. Follow-up of the patients for 12 to 96 months after the operation showed successful immediate implantation in all the cases. After the operation, the changes of the mean alveolar ridge heights on the buccal, palatal, mesial, and distal sides of the patients were 0.8069±0.6253 mm (=1.2904, >0.1), 0.5272± 0.3331 mm (=1.5836, >0.05), 0.5416±0.4048 mm (=1.3379, >0.05), and 0.5172±0.3874 mm (=1.3351, >0.05), respectively; the change of the alveolar ridge width was 0.5522±0.4381 mm (=1.2604, >0.1) mm. The dimension of the alveolar bone underwent no significant changes after the operation in these patients.@*CONCLUSIONS@#Immediate implantation in the maxillary extraction socket with a poor bone quality can avoid damages to the sinus and achieve good outcomes with such advantages of less trauma, full use of the innate gingiva and alveolar ridge, and well preserved morphology of the alveolar ridge as compared with delayed implantation.

ABSTRACT. Introduction: autologous bone grafts for maxillary sinus floor lifting are widely accepted to reconstruct alveolar ridge defects; however, there are donor sites that have not been fully explored and can be valid alternatives for this type of procedures. The objective of this study was to evaluate the behavior of tibia autologous grafts compared with freeze-dried homologous bone in maxillary sinus floor lifting. Methods: prospective, controlled, randomized study in 11 patients requiring maxillary sinus elevation. Panoramic radiographs were taken in three different moments (pre-surgery, immediately after surgery, and 6 months post-surgery) in the two groups (tibia and freeze-dried), measuring the alveolar ridge height in the posterior maxilla. Bone biopsies were taken in the grafted area 6 months after the procedure. Results: A significant reduction in bone height was found in the group grafted with freeze-dried bone. The group grafted with tibia autologous bone showed greater stability between baseline and 6 months after surgery. The histological sections showed equal conditions between the two groups. Conclusion: tibia bone shows greater stability in the evaluated period in terms of the height obtained in maxillary sinus floor lifting procedures, with clinical and histological characteristics suitable for the placement of implants. This study should be complemented with a larger sample to provide more representative results that can be applied to the general population.


RESUMEN. Introducción: los injertos de hueso autólogo para elevación del piso del seno maxilar son ampliamente aceptados para la reconstrucción de defectos en el reborde alveolar; sin embargo, existen sitios donantes que no han sido debidamente explorados y que pueden representar opciones válidas para este tipo de procedimientos. El objetivo de este estudio consistió en evaluar el comportamiento de los injertos autólogos de tibia, comparados con injertos homólogos de hueso liofilizado en la elevación de seno maxilar. Métodos: estudio prospectivo, controlado, aleatorio, que incluyó a 11 pacientes que requerían elevación del seno maxilar. Se tomaron radiografías panorámicas en tres momentos (prequirúrgico, posquirúrgico inmediato y posquirúrgico a 6 meses) en los dos grupos (tibia y liofilizado). En estas se midió la altura del reborde alveolar en el maxilar posterior. Se tomaron biopsias de hueso en la zona injertada 6 meses después del procedimiento. Resultados: Se encontró una disminución significativa de la altura ósea en el grupo injertado con hueso liofilizado. El grupo injertado con hueso autólogo de tibia presentó mayor estabilidad entre el periodo de la cirugía y 6 meses después. En los cortes histológicos se evidenció igualdad de condiciones entre ambos grupos. Conclusión: el hueso de tibia muestra mayor estabilidad en el periodo investigado, en términos de altura obtenida en procedimientos de elevación de piso del seno maxilar, con características clínicas e histológicas adecuadas para la colocación de implantes. Este estudio debe complementarse con una muestra mayor para aportar resultados más representativos aplicables a la población.

This work aims to present the autologous leukocyte and platelet-rich fibrin as a treatment alternative for the sinus mucosa perforation during sinus floor elevation surgery. This case reports on how to obtain the autologous concentrate, the surgical procedure with the lateral window technique, the critical-size perforation repair with leukocyte platelet-rich membranes, as well as the results. In this situation, the autologous leukocyte platelet-rich fibrin demonstrated to be an efficient method to treat a large sinus mucosa perforation.

Este relato de caso clínico descreveu o uso do beta-tricálcio fosfato para levantamento do seio maxilar e instalação do implante dentário. Uma paciente de 38 anos de idade apresentou-se com ausência dos elementos 26 e 27. Após a remoção da parede vestibular e levantamento do assoalho sinusal, uma membrana de colágeno foi interposta antes da deposição do biomaterial (granulação 1-2 mm). Oito meses depois, uma trefina foi usada para biopsia, e dois implantes dentários foram instalados. Os resultados histológicos mostraram a formação de tecido ósseo sadio. Mais tarde, as coroas defi nitivas foram confeccionadas e instaladas. O beta-tricálcio fosfato (Cerasorb) mostrou-se satisfatório como material para enxerto, mesmo sem o uso de osso autógeno concomitante


This case report describes the use of beta-tricalcium phosphate for maxillary sinus lifting and dental implant placement. A 38 years-old patient presented with the absence of the elements 26 and 27. After removing the buccal bone wall for sinus floor lifting, a collagen membrane was adapted before the biomaterial (1-2 mm granules) being inserted. Eight months later, a trephine bur was used for biopsy, and two dental implants were installed. Histological results showed the formation of healthy bone tissue. Later, the definitive crowns were made and installed. The betatricalcium phosphate (Cerasorb) was satisfactory as a biomaterial for grafting, even that no concomitant use of autogenous bone has been performed

ABSTRACT

Objetivo: avaliar por meio de revisão sistematizada os fatores relacionados à taxa de sobrevivência dos implantes, na técnica de elevação atraumática do seio maxilar (LASM) e a influência das variáveis: técnica, tamanho dos implantes e altura óssea remanescente (AOR). Material e métodos: realizou-se pesquisa de artigos sobre as técnicas de LASM publicados na base de dados PubMed entre junho de 2008 e julho de 2014, com amostra mínima de dez pacientes e follow-up mínimo de seis meses. Estes foram divididos em três grupos: levantamento do seio maxilar com osteótomos sem enxerto ósseo (LSMO), e com enxerto ósseo (LSMOE) e levantamento do seio maxilar com brocas e enxerto (LSMBr). A sobrevivência do implante foi considerada como desfecho primário. Resultados: dos 92 artigos encontrados, apenas 18 foram selecionados, envolvendo 1.228 pacientes e 1.581 implantes. O comprimento dos implantes foi dividido em quatro grupos: 6 mm a 7 mm (8,03%); 8 mm a 10 mm (57,42%); 11 mm a 12 mm (27,21%) e 13 mm a 15 mm (7,34%). A AOR prévia à instalação dos implantes foi dividida em dois grupos (≤ 5 mm, 33,79% e > 5 mm, 66,2%). A taxa de sobrevivência dos implantes nas diferentes técnicas variou entre 93,6% e 100% (LSMO), 90% e 100% (LSMOE), e 92,7% e 96,3% (LSMBr). Quanto ao uso de biomaterial, o Bio-Oss (Geistlich) foi o mais utilizado. Houve maior perda quando da utilização de implantes curtos. Conclusão: o LASM apresenta-se como uma técnica segura com altas taxas sobrevivência dos implantes. Fatores como a técnica de acesso ao seio, AOR e tamanho dos implantes devem ser levados em consideração na execução da técnica.


Objective: to evaluate through a systematic review factors related to implant survival rates regarding the atraumatic sinus lifting technique (AMSLT) and the influence of variables such: technique, implant length, and the remaining bone height (RBH). Material and methods: an electronic search was made at the PubMed from June 2008 to July 2014, with at least 10 patients and six months of follow-up per study. These were divided into three groups: the osteotome maxillary sinus lifting with no bone graft (OMSL-NG), the same but with bone graft (OMSL-G), and maxillary sinus lifting with burs and grafting (MSL-BG). The implant survival was considered as the primary outcome. Results: from the 92 retrieved articles, only 18 were selected, involving 1228 patients and 1581 dental implants. Implant length was divided into four groups: 6-7 mm (8.03%), 8-10 mm (57.42%), 11-12 mm (27.21%), and 13-15 mm (7.34%). The RBH before implant placement was divided into two groups (≤ 5 mm: 33.79% and > 5 mm: 66.2%). The implant survival rates ranged from 93.6%-100% (MSL-BG), 90%-100% (OMSL-G), and 92.7%-96.3% (MSL-BG). The anorganic bovine bone matrix (Bio-Oss, Geistlich) was the most frequent biomaterial. Most losses occurred for short dental implants. Conclusion: The AMSLT is safe with high implant survival rates. Factors such as sinus access, the RBH, and implant lengths must be taken into account during surgical execution.

BACKGROUND: Implant surgery has become popular with the advance of surgical techniques such as sinus lifting, guided bone regeneration, and block bone graft. However, there were no data about the frequency of bone graft during implant surgery. The purpose of this study was to report the frequency and types of bone graft depending on dental implant patients' profile to complement the database regarding implant surgery. METHODS: The implant operations had been performed from January 2006 to October 2014. The upper and lower jaws were divided into six sextants. A total of 792 sextants were included in this study. Patient information including sex, age, sites, bone graft, and types of bone were investigated. RESULTS: A total of 1512 implants had been placed. Male and female sextants were 421 and 371, respectively (M:F = 1:0.88). Average age was 54.3 (ranging from 20 to 88 years old). Implants were placed in the posterior maxilla (322 sextants, 40.7 %), posterior mandible (286 sextants, 36.1 %), anterior maxilla (127 sextants, 16.1 %), and anterior mandible (57 sextants, 7.2 %). Bone graft was performed in 50.3 % of the sextants. Among the bone grafted sites, sinus lifting with lateral approach (22.1 %) and guided bone regeneration (22.7 %) were performed most frequently. CONCLUSIONS: Bone graft in implant surgery was necessary to augment defects. More than half of the sextants needed bone graft for implant installation.

ABSTRACT

AIM: This study aims to evaluate the results of 10 cases of maxillary sinus lifting using synthetic resorbable hydroxyapatite (osteogen ®) as a filling material. MATERIAL AND METHODS: The surgeries were performed in patients who presented bone resorption in this region. The results were compared radiographically six months after the surgery through initial and final radiographs analysis. RESULTS: The vertical bone height gain was on average 1200.4%, when compared to the initial bone edge. CONCLUSION: The formation of bone tissue in that region had made possible future prosthetic rehabilitation.

ABSTRACT

O objetivo deste trabalho foi analisar, por meio de imagens radiológicas, a regeneração óssea nas regiões posteriores de maxila atrófica após procedimento de levantamento de seio maxilar com utilização, em diferentes sítios cirúrgicos, de enxerto ósseo autógeno e enxerto ósseo xenógeno (Bio-Oss). No procedimento cirúrgico, a região anterior foi enxertada com osso autógeno em bloco, proveniente do ramo mandibular direito; o seio esquerdo foi levantado com osso autógeno proveniente do ramo mandibular esquerdo; e o seio direito foi preenchido com osso xenógeno (Bio-Oss). Posteriormente, foram instalados os implantes e a prótese tipo protocolo após osseointegração. Após acompanhamento de dois anos, verificou-se aumento da densidade óssea nos sítios enxertados, bem como estabilização dos implantes em harmonia com a prótese.


The aim of this paper was to analyze through radiological images the bone regeneration at posterior atrophic maxillary regions after sinus lifting in different surgical sites using autogenous and xenogeneic bone grafts (Bio-Oss). On surgery, the anterior region was grafted with block autogenous bone from the right mandibular ramus; the left sinus was lifted with autogenous material from the left mandibular ramus; and the right sinus area received a xenogeneic material. After, dental implants were placed and a fixed total prosthesis was delivered after the osseointegration period. The 2-year follow-up images demonstrated an increase on the density at the grafted sites, as well as implant stability and adequate prosthetic integration.

ABSTRACT

O objetivo deste estudo foi apresentar um caso clínico, suportado por achados tomográficos e histológicos, de regiões enxertadas com HA+β-TCP (hidroxiapatita + beta fosfato tricálcio, BoneCeramic) ou osso bovino inorgânico liofilizado (Bio-Oss), em um procedimento de elevação bilateral da mucosa do seio maxilar. A necessidade de enxerto ósseo, para viabilizar a instalação de implantes na maxila posterior foi comprovada pela altura óssea residual inferior a 5 mm, bilateralmente, avaliada por tomografia computadorizada de feixe cônico em paciente edêntulo total, usuário de prótese total convencional. Após a osteotomia de acesso ao seio maxilar e o descolamento da mucosa sinusal, foi realizado o preenchimento da cavidade com substituto ósseo bovino inorgânico liofilizado (Bio-Oss) na maxila direita e com HA+β-TCP (BoneCeramic) na maxila esquerda. Nova tomografia computadorizada foi realizada oito meses após o procedimento de enxertia, e demonstrou ganho de tecido mineralizado em altura nas regiões enxertadas, compatível com a instalação de implantes. No ato da instalação dos implantes foram obtidas biópsias das áreas enxertadas com auxílio de broca trefina. A análise histológica demonstrou que, nas áreas em que ambos os enxertos foram realizados, havia remanescente do biomaterial sempre rodeado por tecido ósseo neoformado e tecido conjuntivo mole. Entretanto, a análise qualitativa subjetiva sugere uma tendência maior de presença de material mineralizado quando o osso bovino inorgânico liofilizado (Bio-Oss) foi utilizado. Dentro do limite deste estudo, pôde-se concluir que ambos os materiais, quando utilizados em procedimentos de enxertia para aumento ósseo no assoalho do seio maxilar em humanos, foram capazes de promover o ganho em altura óssea, apresentando comportamento clínico e tomográfico semelhantes depois de oito meses.


The aim of this study was to report a clinical case, supported by CBCT and histological findings, of regions grafted with HA + β-TCP (hydroxyapatite + beta tricalcium phosphate, BoneCeramic) or inorganic bovine bone lyophilized (Bio-Oss) for bilateral maxillary sinus lifting. The need for bone grafting in the posterior maxilla was confirmed by residual bone heights of less than 5 mm, in a totally edentulous patient, wearing a conventional total prosthesis. After osteotomy and detachment of the sinus mucosa, the formed cavities were filled with Bio-Oss (right side) and BoneCeramic (left side). A new CBCT scan was performed eight months after grafting procedure, and demonstrated gain of mineralized tissue height in the grafted regions, consistent with placement implant. After installation of the implants, biopsies were retrieved from the grafted areas with the aid of a trephine bur. Histological analysis demonstrated that, remaining biomaterial particles were always surrounded by newly formed bone and soft tissue in both sides. However, the subjective qualitative analysis suggests the presence of a larger trend of mineralized material when lyophilized inorganic bovine bone (Bio-Oss) was used. Within the limits of this study, it can be concluded that both materials were able to promote significant gain in bone maxillary height after eight months.

A técnica do levantamento de seio maxilar permite a instalação de implantes em deficiências de altura na região posterior da maxial. Os autores apresentam 20 levantamentos de seio maxilar realizados com associação de osso autógeno e de osso xenógeno inorgânico bovino (Gen Ox).


The sinus lifting technique allows implant placement at posterior maxillary regions with defficient bone heights. The authors present 20 cases where the autogenous material was mixed with an anorganic bovine bone in a 1:1 ratio.

ABSTRACT

A Implantodontia é uma especialidade que apresenta grande previsibilidade na reabilitação de pacientes desdentados posteriores de maxila. A perda dentária precoce resulta em remodelação acentuada dos maxilares. A elevação da membrana do seio maxilar com instalação de implantes dentários é uma técnica previsível descrita em 1980. Desde então, técnicas, materiais de preenchimento e o gerenciamento de complicações vêm sendo estudados, a fim de proporcionar diretrizes eficazes na reabilitação deste grupo de pacientes. Este estudo teve como objetivo avaliar clínica e radiograficamente dez pacientes que necessitaram de elevação da membrana do seio maxilar para instalação de implantes dentários e próteses implantossuportadas. Inicialmente, realizou-se uma análise retrospectiva dos prontuários para a obtenção de dados relacionados a possíveis complicações pós-operatórias. Estes pacientes foram acompanhados também em avaliação clínica e radiográfica em zero e 180 dias após a cirurgia. Clinicamente e radiograficamente, pôde-se concluir que a técnica cirúrgica do levantamento do seio maxilar com instalação imediata de implantes apresenta resultados satisfatórios, podendo ser considerado um procedimento seguro, pois 90% dos pacientes alcançaram previsibilidade, de um total de 86,96% dos implantes instalados. Ressalta-se, ainda, a importância do conhecimento anatômico da região, a técnica cirúrgica apurada e o acompanhamento pós-operatório dos pacientes.


Implant dentistry is a dental specialty which presents great predictability in the rehabilitation at posterior, partially edentulous maxillary areas. Early tooth loss results in significant jaw remodeling. The maxillary sinus lifting followed by implant placement is a predictable technique initially described in 1980. Since then, several different techniques have been investigated varying filling materials and the management of complications in order to provide effective guidance in the rehabilitation of these patients. The current study evaluated ten patients who underwent sinus lifting before implant placement and crown installation. First, a retrospective analysis of the medical records was conducted to obtain information about possible postoperative complications. Clinical and radiographic analyses were performed at baseline and 180 days after surgeries. The sinus lifting with immediate implant placement provided satisfactory outcomes and can be considered a safe procedure. Treatment predictability was demonstrated in 90% of patients and for 86.96% of implants placed. It is important to highlight knowledge of anatomical structures at this area, the use of delicate surgical techniques, and strict patient follow-up.

ABSTRACT

Este artigo relata um caso clínico de reabilitação implantossuportada de uma área posterior de maxila, com extensa reabsorção óssea e pneumatização do seio maxilar. Paciente do sexo feminino, submetida à instalação de dois implantes Replace Select Tapered (NobelBiocare AB, Gotemburgo, Suécia) na região dos dentes 15 e 16 e concomitante levantamento de seio maxilar. O seio maxilar foi preenchido com osso xenógeno liofilizado (Bio-Oss, Geistlich Pharma AG, Wolhusen, Suíça). Após o período de seis meses de cicatrização foi realizada a confecção da prótese metalo-cerâmica fixa parafusada. Com a utilização deste substituto ósseo e implantes, foi possível diminuir o tempo de tratamento e realizar uma cirurgia com menor morbidade e desconforto para a paciente.


This article reports a clinical case of an implant-supported oral rehabilitation in the posterior maxillary with extensive bone resorption and sinus pneumatization. Two Replace Select Tapered implants (NobelBiocare AB, Gotemburgo, Suécia) were placed in a female patient replacing teeth 15 and 16 immediately after the sinus lifting procedure. The Sinus was filled up (grafted) with xenograft lyophilized bone (Bio-Oss, Geistlich Pharma AG, Wolhusen, Suíça). After six months of the healing process a screw-retained metalloceramic prosthesis was fabricated. With this protocol of treatment by using bone substitute and immediate implant placement, there was a reduction of treatment time and surgical performance with less morbidity and discomfort for the patient.

ABSTRACT

Sinus lifting was introduced for prosthetic reconstruction in 1960 decades by Boyne and is specifically used for implant surgery in maxillary posterior region with insufficient bony height and has been indicated for reconstruction of oro- antral and palato-alveolar cleft. We report that subtotal maxillectomy using sinus lifting and skin graft in mild maxillary cancer cases results in good prevention of oro-antral and oro-nasal fistula by preserving intact maxillary sinus mucosa and improves better functional outcome after maxillectomy.
